MATCH REPORTS & Game Cards
All game cards will be kept and/or returned to the Center Referee upon the conclusion of the game. Once the cards are completely filled out upon the game's conclusion, they need to be initialed by each coach in agreement with the final scores. All cards are then deposited in the AYSO mailbox that is a fixed to the Lexington/Arnold fence next to the lock ups. Game cards are kept for a period of 4 weeks in case of any discrepancies. Please contact either the RRA or ARA if you have any questions.
Please fill out a game/match report for all injuries resulting in a player(s) leaving the match and did not return, misconducts, including any and all issues/concerns, on the day of the game. These should be done in MatchTrak if you are a registered AYSO Referee with MatchTrak access (see instructions below for support). If you prefer to provide a paper written report, you can get one below or at the field. Please send the completed report to the Regional Referee Administrator (RRA) within 24 hours of the game day.
